Jeremiah’s Inn Inc. assists over 100 men each year in making the necessary changes to their thinking and behavior that supports a recovered lifestyle. Recovery is so much more than staying abstinent from drugs and alcohol. At JI we use the “Social Model” experiential modality of treatment along with the American Society of Addiction Medicine’s (ASAM) criteria to formulate a comprehensive, individualized treatment plan with each patient. This evidence-based approach highlights an individual’s strengths, while focusing on challenges they may face while working toward their personal recovery goals.
1. PURPOSE To define eligibility criteria for Jeremiah’s Inn Social Model Men’s Recovery Home in accordance with the ASAM 6 Dimensions of Change, Level 3.1 Co-Occurring Capable criteria.
2. DEFINITIONS
Dimension I – Acute Intoxication and/or Withdrawal Potential – The Client meets the admission criteria for this dimension as evidenced by one or more of the following indicators:
Dimension ll – Biomedical Conditions/Complications – The client meets the admission criteria in this dimension when their biomedical condition is stale or sufficiently stable to allow the client to participate in the essential components of a low intensity residential recovery program.
Dimension lll – Emotional/Behavioral/Cognitive Conditions and Complications – The client meets the criteria in this dimension when their behavioral condition is stale or sufficiently stable to prevent behaviors which put themselves or others at risk. The client meets all criteria under this dimension if he is able to:
Dimension lV – Readiness to Change – The client meets the admission criteria in this dimension as evidenced by the following indicators:
Dimension V – Relapse/Continued Use/Continued Problem Potential – The client meets the admission criteria in this dimension if he demonstrates a high risk for relapse as evidenced by at least one of the following indicators:
Dimension Vl – Recovery Environment – The client meets the criteria for admission in this dimension if he demonstrates a need for structure and support to address early blocks to recovery. This need is evidenced by one or more of the following indicators:
Admission Diagnostic Criteria: Diagnosis – This resident meets the criteria for alcohol and/or drug abuse/dependence as defined by the Diagnostic and Statistical Manual of Mental Disorders (DSMV) or other commonly accepted diagnostic criteria. The current diagnosis may be modified by the qualification, “in remission”. All applications for admission are assessed on an individual basis.
